Wellness Recovery Action Plan (WRAP) in New Hampshire
The Wellness Recovery Action Plan®, or WRAP®, is a structured prevention process that helps you stay well and avoid relapse. WRAP: Evidence-Based Practice Explained
WRAP is a wellness and recovery system you design for yourself through self-determination. Mary Ellen Copeland developed it in 1997 to help people build personalized plans to get well, stay healthy, and shape their lives the way they want.
It earned recognition as an evidence-based practice and was previously listed on SAMHSA’s National Registry of Evidence-Based Programs and Practices, which has since been retired. It remains widely used in behavioral health programs and continues to show benefits for mental health and overall wellness.
How a Wellness Recovery Action Plan Works
WRAP began in mental health recovery but is now used in addiction treatment, too. It’s best supported by mental health research, yet many in recovery find it helps them stay balanced and sober. Its core six parts include:
Daily Maintenance Plan
Figure out what wellness means in your life. What keeps you feeling steady? What do you need each day to stay on track? Sleep, meals, exercise, and time with people who support you go here.
Triggers
Write down events or situations that upset you or spark urges to use substances. Knowing your triggers in advance means you can plan how to handle them rather than getting blindsided.
Early Warning Signs
Learn to catch subtle signals that sobriety might be slipping. Maybe you’re sleeping less, withdrawing from people, or noticing shifts in your thinking. Noticing these early gives you a chance to course-correct.
When Things Are Breaking Down
Map out what to do when warning signs intensify and you’re heading toward a crisis. This includes emergency contacts, actionable steps to follow, and more to remove guesswork that might trip you up.
Crisis Planning
Document what others should do if you can’t make sound decisions or keep yourself safe. This includes who to call, what medications you take, what helps you calm down, and what makes things worse.
Post-Crisis Plan
After a crisis, take time to reflect on what happened. The post-crisis plan helps you identify what to do differently, update your wellness tools, and reconnect with your support system to get back on track.
WRAP Wellness Recovery at Granite Recovery Centers
We weave Wellness Recovery Action Plan approaches into treatment so you have practical tools for staying sober long-term:
Detox and Residential Treatment
You start building your WRAP plan while you’re in our inpatient mental health program. This is when you figure out what actually supports your wellness and what threatens it.
PHP, IOP, and Outpatient
You keep refining your plan as you ease back into daily life, from PHP straight to standard outpatient. The framework helps you handle triggers and hold onto progress.
Dual Diagnosis
We help individuals who need treatment for one or more mental health conditions or substance abuse. During dual diagnosis, WRAP tackles how they inform each other.
Sober Living and Aftercare
Your wellness strategy comes when you start sober living or finish a program. You’ll update as you discern what works in real life versus what worked in a controlled space.
Why the WRAP Plan Works for Relapse Prevention
The Wellness Recovery Action Plan gives you practical tools to notice changes in your well-being early and respond before stress or cravings take hold:
Personalizes your recovery:
Your plan reflects your life, your triggers, and your support system. What works for someone else might not work for you, and WRAP accounts for that.
Catches problems early:
Relapse rarely shows up out of nowhere. Wellness recovery trains you to notice small changes (mood dips, sleep problems) before they snowball.
Provides control over situations:
You’re not scrambling when cravings hit or stress piles up. You’ve already mapped out your response, which cuts down on panic and impulsive decisions.
Creates accountability:
Your plan names the people supporting your recovery. When warning signs pop up, you know exactly who to lean on instead of trying to tough it out alone.
Builds on evidence:
WRAP has over 20 years of research showing it can improve well-being and self-management, helping many people feel more stable in recovery.
Who Benefits From a Recovery Action Plan?
WRAP helps anyone serious about recovery, especially:
- Individuals wanting more say in how their recovery unfolds
- People wanting concrete, daily strategies to prevent relapse
- Those juggling mental health issues alongside substance use
- People who’ve relapsed before and need stronger safeguards
- Anyone moving from residential treatment back to independent living
Reviews and Success Stories
-Courtney
The most caring recovery team that works there. Awesome accomodations. All around though really good program. First time to rehabilitation center and was my favorite stop on my recovery road. Glad I choose to go there. Hands down best place to go for real recovery survivors 100%.

Frequently Asked Questions
Do I create my WRAP plan alone?
You lead the process, but you don’t do it alone. Trained WRAP facilitators guide you through each step so your plan stays true to the model and reflects what genuinely helps you stay well.
How long does it take to complete a Wellness Recovery Action Plan?
Most people spend several weeks developing their first plan. It’s not a one-and-done assignment. You’ll revisit and revise it as you learn what actually prevents relapse versus what you thought would work.
Is WRAP only for people with mental illness?
No. It started in mental health circles, but now it’s used for substance use disorders, chronic health problems, major life changes, and virtually any situation where you want to prevent things from escalating into unmanageable moments.
What if my triggers or warning signs change over time?
They will change. Your WRAP plan should shift with you. Regular check-ins help you update what to watch for and how to respond based on where you are now, not where you were six months ago.
Can family members help with my WRAP plan?
Absolutely. Your crisis planning section can name family members or close friends who know what helps when you’re struggling. Some people share their whole plan with loved ones so everyone’s on the same page about supporting recovery.
How is WRAP different from regular relapse prevention?
Wellness Recovery Action Plans cover more ground. Standard relapse prevention tends to zero in on not using substances. WRAP looks at your whole life: daily routines, relationships, self-care, meaning, and purpose. It’s less about avoiding something bad and more about protecting something good.
